Во-первых шла Северная война 1700-1721 гг. 
Население набирали в армию, учитывая тогдашние армейские порядки и уровень военной медицины, подохнуть под палками капралов во время обучения или от ранения после сражения можно было запросто. 
Из 100 рекрутов до полка добиралось 90, остальные ...либо помирали от плохого питания либо от наказаний. 
После сражения из 100 раненых выживало дай бог если 50, и это считалось неплохо. 
Во-вторых, Петр Первый сгонял крестьян на строительство Петербурга, от непосильного труда и плохой кормежки смертность там была высокой. 
В-третьих: реформы Петра Первого требовали денег, а их собирали с населения. За невыплату налогов в те времена можно было и на каторгу загреметь, а то и на виселицу. 
Так что "петровские преобразования" конечно хорошо, но жить в то время было трудно. Выживали далеко не все.
