Past Continuous:

1. He was watching TV when the phone rang. TV was being watched by him when the phone rang.
2. We were studying at the library during the afternoon. Studying was being done by us at the library during the afternoon.
3. They were playing basketball in the park when it started raining. Basketball was being played by them in the park when it started raining.

Past Perfect:

1. She had finished her work before the meeting started. Her work had been finished before the meeting started.
2. They had already eaten lunch when we arrived. Lunch had already been eaten by them when we arrived.
3. He had completed the project by the time the deadline approached. The project had been completed by him by the time the deadline approached.

Passive voice is formed by using the auxiliary verb "be" in the appropriate tense and the past participle of the main verb.
